To request a free Bonus Card without making a purchase, hand print your name, complete address, and email address (required) on a postcard, and mail to: Applebee’s Mystery Value Bonus Card Request, P.O. Box 1579, Palatine, IL 60078-1579. Requests must be postmarked between October 26, 2015 and January 4, 2016 and received by January 11, 2016. You will receive your Bonus Card via return email within thirty (30) days of receipt of your request. All requests/materials submitted become property of Sponsor and will not be returned or acknowledged.
You will automatically receive a Mystery Value Bonus Card (“Bonus Card”) when you make a purchase transaction valued at $50.00 (or more) of Applebee’s gift cards during the Play Period at an Applebee’s restaurant or online at applebees.com. If purchasing at an Applebee’s restaurant, you will receive your Bonus Card(s) at the restaurant. If purchasing online at www.applebees.com, you will receive your Bonus Card(s) via return email.
